There are two methods available for viewing, downloading, or printing a questionnaire.
As a digital first solution, please be aware that the format of printed questionnaires is sub-optimal.
To view, download or print a questionnaire
The following steps provide access to a blank copy of the questionnaire for a specific product.- From the top bar click on More > select Products.
- Search for the product for which you wish to access a questionnaire.
- Click on Print Q'naire.
To view, download or print a client's questionnaire
The following steps provide access to a copy of an individual client's questionnaire. Client personal details and any completed questionnaire data will be included.
- From the top bar click on Schedule.
- Search for the client's appointment date.
- From the clinic schedule, click on the three dot menu to the right of the client's name.
- From the drop down menu, select Print questionnaire.
