If the medical programme you are participating in has been set up with a pre-assessment questionnaire, you will be able to access and complete this via your online account.
Once you have logged into your online account, you will be presented with your account.

Click on Complete questionnaire.
Review and confirm personal details are correct > select No, update details or Yes, correct.
- Begin your pre-assessment questionnaire.
- Your data will be autosaved as you go and will be saved if you do not have time to complete in one sitting.
- Click Submit once all questions are complete.
